Athletic Clearance Forms (previously called Ticket to Play)
NEED to be turned into Poway Athletics Office before tryouts.

Failure to submit athletic clearance by the due date may delay and/or prohibit the athlete from trying out.
Forms dropped off after the due date will need at minimum one week to process.

Please create an account if you don't have one and follow the process to get to a confirmation page.

☐      completed PUSD Health History and Physical Exam forms.
Physicals can only be accepted from a California Licensed Health Care Provider, are valid for 12-months, and cannot expire during the selected sports season.
