Uniform Distribution:  Thursday, November 18th 

Please complete the PHS Girls Soccer Uniform Usage Agreement and provide a written check for the Uniform Deposit (Novice/JV $100, Varsity $200) at uniform pick up. 

 

This check will be uncashed and returned to you/destroyed upon uniform return. PLEASE write check out to: PHS Girls Soccer Boosters

 

We must receive the Usage Agreement and Uniform Deposit before we can issue your player a uniform kit.
